Can you try this after you've done:

cd /usr/src
mergemaster

as per /usr/src/UPDATING's suggestions after - or before - every
buildworld/installworld cycle?

Also, can you make sure that you have cvsup'd /usr/src/etc (that's where
the new pam.conf lives) along with the OpenSSH source?
