And an experimental version that uses VT100/ANSI escape sequences to 
remove the in-band quote marks that many readers will type, and 
highlight those sections with out-of-band underlines:

I have no evidence, but suspect the number of people using non-VT100, 
non-ANSI terminal emulations is very small, and those people will 
probably not be bothered by escape sequences that do not render 
correctly.
