I suspect that a modem with that vintage, there will be some jumpers that allow you to configure it to actual ports and IRQs. Particularly, I suspect that this is an ISA card, which makes setting the ports and IRQs yourself even more sensible. I have done with with every ISA card I every had (except for a SB16 that didn't have jumpers), and I have never had any problems with them (except for the SB16). I did this even under Windoze. If you are dual booting with Windoze on the other half, you run the risk of Windoze changing the settings and not telling you about it (cf. SB16 above) resulting in hours of frustration later.
