They're in src/UPDATING.  If you follow them carefully then upgrading from
3.4 to 4.0 works fine - I did two machines in the last two weeks.

That being said, if I'd had local access to the machines I probably would
have done a data backup and installed 4.0 from scratch, because it would
have been a nice opportunity to tweak partition sizes, clean out old gunk,
etc.
