Try fdimage 1.5 (May 1997) available at

    ftp://ftp.freebsd.org/pub/FreeBSD/tools/fdimage.exe

and most mirror sites.  There was a bug in the initial release of
fdimage (AFAIK still distributed on the 2.2.1 CD-ROM) which caused
this problem.

If version 1.5 doesn't work for you, please send debug output

    (eg. fdimage -d boot.flp a:)
