Use a refuse file.  Here's an excerpt from cvsup(1):

     As another example, to receive the FreeBSD documentation files without
     the Japanese, Russian, and Chinese translations, create a refuse file
     containing the following lines:

           doc/ja*
           doc/ru*
           doc/zh*
