I have tried with these:
{0x40001813, "Creatix V.90 HaM Modem", 0x10}
{0x40001813, "Creatix V.90 HaM Modem", 0x14}
If I do a boot -v I get this:

I also have added to my kernel configuration file option
PCI_ENABLE_IO_MODES.
May be, I should add this modem to src/sys/dev/puc/pucdata.c.
Do you have any ideas?
