Your boot log shows this. pci0: <unknown card> (vendor=0x1014, dev=0x0057) at 5.0 irq 11 This is your modem. This means your modem is an winmodem. A winmodem is specially build modem for the ms/windows market. It's cheaper because it does not have an onboard controller. The controller function is provided by the modem software you install into ms/windows. FBSD does not work with winmodoms. Save your self a lot problems and use an external modem. I use zoom model 3049L.
