A brief update as a few reports of the above problem were made:

Tried changing the drives to <SEAGATE ST34572W> and upped the timeout
values in the scsi_da driver and the problem still persisted;   then
swapped the <Adaptec 3940 Ultra SCSI host adapter> rev 0 for an
<Adaptec 2940 Ultra SCSI adapter> Rev C and the problem has not returned.
