I wouldn't consider this a 'real checkout' of a previous version.  If
I want a previous version, I want it exactly - no changes, to be able
to compare with others etc.  Changing directory layout is different.

No, CVS needs understanding of directories and that files can move.
There has been discussion of adding this at cvs-info list, but I don't
think anything has come of it so far.

In fact, CVS probably need a major change of the database interface,
to allow use of different databases for different purposes - I'd be
willing to sacrifice disk space for speed, but I'm not certain
everybody else would.
