Hi, I want to report having duplicated this exact same error on BHYVE virtual
machine with PCI passthrough for the USB.

HARDWARE:  TRX40 Tachi with Ryzen 3960x (amd64)
HOST SYSTEM:  12.1-RELEASE-p8
BHYVE Versions tried:  11.4-RELEASE, 12.1-RELEASE, 12.1-STABLE, 13.0-CURRENT

BEHAVIOR:  
Without PCI passthrough, the host xhci controller operates normally with no
errors.  After rebooting host, the bhyve VM can be started, only once, with no
USB xhci errors.

However, if the VM is power cycled without cycling the host, I get the same
sequence of errors reported by Mark, in a continuous loop, and inserted USB
devices are undetected. 

Although the screen is impractical to use due to error messages, it is possible
to login.  I can SSH into the VM and otherwise use it normally.  After 3-5
minutes, the loop stops, the VM detects devices, and I can access them.

SPECULATION
Problems with PCI reset?  Perhaps my hardware doesn't support reset or
re-initialization of the device without reseting all devices?
