Be sure to actually visually inspect your brand-new-from-the-case CDRs. I find that roughly 1 in 10 CDRs have a few bits of plastic "dust" on them, even with quality brands. The bits are easily blown off, but some of the pieces are probably big enough (relative to the tiny laser beam) to ruin a sector of the disk if you don't bother to remove them...
