I am not sure if this is where to sent this but...
I have a USRobotics Courier V.Everything ISA modem and adding the
following line to /ys/isa/sio.c makes it work great:
{0x09007256, NULL}, /* USR0009 - USRobotics Courier V.Everything */
output of pnpinfo is like this:
