I would like to commit the patch in standards/78907 to HEAD with the
intent of MFCing to RELENG_5.  This patch is required to compile Sun
Grid Engine 6.  Are there any objections to it?

The summary is that it extracts the pthread types from pthread.h into
sys/_pthreadtypes.h and includes that in both sys/types.h and pthread.h.
