I've developed a patch for the same feature in LibAlias (tested with IPFW but presumably applies to natd/pppd too) on bug 219918, which you might want to look at first, as it's much shorter and simpler than this one, only about 200 lines long. Also my tests show IPFILTER already does endpoint-independent mapping, as does "iptables" in Linux.
