I've tried cleaning the drive, a new DAT > tape, but no joy. Is this a new drive? MEDIUM ERROR seems to indicate that there are problems in the physical process of writing data to tape. If using another tape did not help then maybe the drive is just dying of old age. Lasting a long time is not one of the strong sides of DAT drives. Another thing to check out is the SCSI chain.
