For the specific examples of ports man pages, I think something like:
&man.ports.net.isc-dhcp3.dhcpd.5;
is going to be necessary. This could then expand to something like:

<refentrytitle>dhcpd</refentrytitle>
<manvolnum>5</manvolnum>
</citerefentry><footnote>
<para>From <filename class="port">net/isc-dhcp3</filename></para>
</footnote>
