It usually indicates a cabling or termination problem. If the message is
"timed out in {datain,dataout,command} phase", the cause is often the same
-- cabling or termination.
If you see a "timed out while idle" message, that often points to buggy
drive firmware, but in some rare cases it can just mean that there's a
timeout set too short.
In any case, check your cabling and termination.
