This seems related. On a server I upgraded to 10.1-RELEASE (via buildworld),
and then updated to the latest patch level, I get this in dmesg after boot:

Trying to mount root from ufs:/dev/mirror/root [rw]...
WARNING: / was not properly dismounted
WARNING: / was not properly dismounted
