I had a Startech CMD649-based controller (ATA/100 I believe) that I recall worked rather well, but I didn't have it for long, and there seems to be some general CMD64x unhappiness floating around... But they seem to be more of a full-fledged IDE controller than the Promise cards. And sure, you could use PIO4; that's what I did when I used the Promise. It worked at 16x, anyway. YMMV I suppose.
