I had this problem as well, and the problem wasn't the card, but the fact that my pcmcia needed to have an IRQ assigned to it. I did this by changing my pcic line in my kernel config to read : device pcic0 at isa? irq 5 port 0x3e0 iomem 0xd0000 and recompiling. There are apparantly other ways of doing this.
