Yep, that's it -- almost. It is described as starting at the index hole, but it's actually starting at the first ID field that is found. The consequence is that the first sector's worth of data is indeed in the correct bit synchronization. The remaining sectors aren't, they are being delivered in the bit synchronization as they happened to pass under the head. I.e., they can be bit-shifted and/or inverted.
