The error: > swap_pager: indefinite wait buffer: bufobj: 0, blkno xxx, size: 4096 you're seeing is one i'd normally associate with hardware failing. Like it can't read or write to a cluster. The next steps I'd take would be to install smartmontools and run a long test on the disk.
