I wonder if your pc164sx boots fine on one of scsi disks with 2940uw
before connecting a Yamaha cd-r writer.
I have got alphapc164 and pc164lx boxes in my lab, and they refuse
working well with 2940uw.

You can probably check setting of the SRM variables, like
pka0_fast
pci_parity
etc.

Is Yamaha a fast scsi device?
