I'm almost convinced that the logic in cron for handling multiple system
crontab files is completely wrong and works only by chance, but I haven't
managed to pin down what the conditions are to make it fail.

Can you show the output of

ls -lT /etc/crontab /etc/cron.d /usr/local/etc/cron.d

and the output of

ls -f /usr/local/etc/cron.d
