Can you confirm if this is still a problem? I cannot reproduce this
on 4.11:

Using the MD5 algorithm to compute response.
Sequence number 0 is not positive.

So there is now way to get a negative seq-#
This is with pam_opie. You also mention S/Key, are you maybe mixing both?
