Take a look at: 
http://www.unix.digital.com/faqs/publications/pub_page/V40D_DOCS.HTM
and look for the title 'Guide to DECthreads' - despite the name, 
pthreads are the main part of the book (The PDF-file printed gives you 
a real nice book)
