I believe the:

ugen0.4: <Unknown > at usbus0 (disconnected)
uhub_reattach_port: could not allocate new device

is where the problem lies. As I mentioned elsewhere there is an iLO5 power
saving setting which smells relevant even though it only talks about the USB
HID
