Some cisco kit runs NTP itself. Plus the IP number it's connecting to belongs to serv000.worldonline.dk, and the whois information shows that IP belongs to Tiscali, so I think what you're seeing is just the way your ISP has configured your router. It's hard to say precisely without knowing more details of the way your system is set up, but I don't think there's any major cause for alarm. There may be some slight misconfiguration causing the NTP packets to get sent through to your internal machines --- but that's harmless if nothing is listening on port 123.
