Obvious choice for the 4 Ethernets would be a 4-port DEC 2114x based cards, available from (at least) Adaptec, Matrox, Znyx and Phobos. AFAIK, the greatest density you can get with an Intel based card is two 82558 based Ethernet ports per PCI slot.
