Trying to test the 1st possible root cause (Having a mix of USB 2.0, USB 3.0
and USB 3.x ?), I ran tests on:

Lenovo Thinkpad W540:

I tried different USB UEFI Setup Configurations, but I didn't see any of the
errors mentioned.

That lead me to the path that it could be a mix of both Possible Root Cause=
