Trying to test the 1st possible root cause (Having a mix of USB 2.0, USB 3.0 and USB 3.x ?), I ran tests on: Lenovo Thinkpad W540: --------------------- Chipset: Mobile Intel QM87 Express (having USB 2.0 and USB 3.0) I tried different USB UEFI Setup Configurations, but I didn't see any of the errors mentioned.
