I had the exact same problem. I was using a Creative Soundblaster PCI 128, 
a nvidia RIVA TNT2 and a S3 ViRGE DX/GX.

I recently replaced those two video cards with a ATI Radeon VE dualhead, 
and the problem went away. So I assumed it was a memory conflict of some 
kind.
