The usual culprate to this problem is permissions.

Check that the uid of the rootweb is > 11 and the gid is > 21.

For virtual webs and user webs, you also need to check:

1. The access.conf has "Options ExecCGI" set for the directory.
2.  If using suexec, is the server running as the correct user that suexec
expects (default www).
