I haven't used this software, so I an mot sure this is what you are
looking for, but you might want to look at <http://www.flams.com/>.
FLAMS is the FastLane Account Management System.  The page states:

"FLAMS is a total account management and billing system designed
for Internet Service Providers. It is web based, runs on
FreeBSD  (running Apache, MySQL, and a proprietary form engine
developed in house)."
