It would be nice if MAINTAINERS was sorted in some manner I comprehended.
:-)  Likewise, it would be nice to constrain the set of entries in the
"notes" column to a fairly standard set, and call it "constraints", with
notes being used for additional commentary.  There would also be the
temptation to make "subsystem" be more structured -- perhaps a
path/file/set/... of some kind, which might work in many cases.  Vis.

src/usr.sbin/mergemaster	dougb	prefer-preapprove
src/contrib/binutils		obrien	preapprove
src/contrib/gcc			obrien	preapprove
src/sys/dev/ata			sos	prefer-preapprove
src/sys/dev/ahc			gibbs	prefer-preapprove
...

(recognizing that some parts of the system live in many directories, ...)

This would make it far easier to take a file and determine if it has
a maintainer.
