My two computers running FreeBSD 9 can't even fdformat the floppy. The
format utility runs and marks all tracks bad. If I abort the process, the
computer panics. I always fdformat a floppy before running DD because I find
that DOS fomatting of the disk does not catch all of the surface errors.

The floppy disks that were marked bad by FreeBSD 9 were successfully
formatted by my laptop that runs OpenBSD 5. I was able to successfully DD
the image from CDROM to the 'bad' floppy disk from the laptop.
