I found some source files in FreeBSD have 4-clause BSDL which is against FreeBSD license policy. Those files were derived from NetBSD before NetBSD moved to 2-clause BSDL.

For example:

src/lib/libc/stdlib/getopt_long.c
src/include/getopt.h
src/usr.bin/gencat/gencat.c
