I'd be tempted to change md(4) to simply ignore BIO_FLUSH in malloc-backed =
MDs,
rather than adding complexity to bhyve.  The second patch looks ok to me.

Should swap-backed MDs also be changed?  What effect should BIO_FLUSH have
there?
