This is most likely a hardware problem. 

What kind of disk drive is that ?

Can you try upgrading to /sys/pci/ncr.c 
from the -stable sources ?
(The error handling and recovery code is 
improved in that version.)

} vm_fault: pager input (probably hardware) error, PID 0 failure
} swap_pager: I/O error - pagein failed; blkno 456, size 4096, error 5

That's surprising, since there are no 
driver visible differences between 
single and multi-user mode ...

Please try to stop the output using 
the SCROLL LOCK key on your keyboard
(as early as possible) and report the
first error message (use the PAGE UP
key to scroll back).
