I reported the same problem in bug report =

work around the problem by using the FreeBSD/alpha 4.11-RELEASE =
installation or mini-installation CD to format the device.

Versions prior to 5.3-RELEASE do not correctly support the Mylex DAC960 =
(KZPSC) array controller.  I have 5.3-RELEASE installed and running =
properly on an AS2000 with a RAID-5 array hosted by the KZPSC =
controller.
