I've got grip working pretty well with 'cdda2wav', which is part of the sysutils/cdrtools port. Grip has cdda2wav in its menu of rippers, I think it got the options right from the start -- I just had to tweak the filename format to my liking. This is with an ATAPI DVD-ROM drive, BTW. It may help to symlink /dev/cdrom to the real device node, since grip defaults to that.
