The entry in pccard.conf is there, but I neglected to mention that I'm also using PAO3-19991011. I couldn't get the card to work without PAO, but I attributed that to my own inexperience. After a week or so of trying without it I finally loaded PAO and things went much more smoothly. I thought since the entry was there it must have worked for someone, so it was just me doing something wrong.
