Depending if you use pfsync or not it's:
   MFC: pf_ioctl.c, 1.20 if_pfsync.h, 1.7 and if_pfsync.c, 1.16-1.19
or MFC: pf.c, 1.35 pfvar.h, 1.12

Pulling just sys/contrib/pf to RELENG_5 should work, AFAIR.
