added many countries, and a few cities
corrected various dialcodes, from 1996 phonebook
NB: many ex-soviet countries with prefix 7 were not added, and others
might have been missed
some ex-yugoslav states are probably missing
Belgrade is both in Serbia and Yugoslavia, while the latter retains
an existence
