At the bottom of that section is this sentence:

    mysql_upgrade was added in MySQL 5.0.19. It supersedes the older
    mysql_fix_privilege_tables script.

5.0.19 hasn't been released yet, but I expect that when it is, and the
port is updated to build it, the command will be available.
