I haven't forgotten about this issue.
Unfortunately, I can not devote as much time as I would like to it.
I revisited it again and I couldn't spot anything new besides what my old patch
was supposed to fix:

I know that George [cc-ed] was going to work on this problem.  Perhaps, he has
something for you to test or debug.
